I love that your examples take me back in a Santayanan moment or two.  (Although I had to check up on the Therac-25 example). Whatever happened to Myers maxim that for testing to succeed, it must find bugs? With IoT, if the expectation is that there will be massive failures, perhaps we will find the flaws before the customers do.
